Combining Science and Art

A breast reconstruction surgical procedure not only restores the appearance of the breasts, but will also help boost the patient’s self-esteem. The objective of the surgeon should be to create a new breast with a desirable shape while ensuring proportion and symmetry.

There may be limitations on the availability of skin. However, the surgeon must employ the best technique to ensure that the outcomes improve the general physical profile and are desirable and natural looking.

At the same time, the surgeon should perform the procedure in a conservative manner which conforms to the principles of science. This provides outcomes that are sustainable, healthy, and enhance the patient’s self-confidence.

The surgeon can achieve these goals through a combination of both science and art to offer the most consummate results in breast reconstruction procedures.

Individualizing the Surgery

Dr. Seify will assess the patient’s personal and anatomical cosmetic requirements to develop an individualized breast reconstruction surgical plan. Rather than adopting an off-the-rack or cookie-cutter approach, Dr. Seify will strive for the most desirable results in the least invasive way.

The judicious Dr. Seify may use breast implants or the flap reconstruction technique depending on the patient’s unique requirements. One goal that Dr. Seify focuses on during a breast reconstruction procedure is to ensure that the incisions are placed in inconspicuous areas or within the natural folds of the skin to achieve minimally noticeable scarring.

Choosing to go ahead with a breast reconstruction cosmetic surgery procedure is a life-altering Questions to Ask Your Breast Reconstruction Surgeondecision. The patient should make this choice after careful deliberation and after clarifying all her doubts and concerns from the breast surgeon.

The patient will have an opportunity to ask questions at the time of pre-op consultation. Board certified plastic surgeon Dr. Hisham Seify provides breast reconstruction surgery to patients in Newport Beach, Orange County, CA, and surrounding locations.

List of Questions

The patient should preferably prepare a list of questions in advance so that none of the pertinent questions are missed out. The surgeon will make sure to provide accurate and detailed answers to the questions and help her make a well-considered decision.

  • Are you an ABPS board certified plastic surgeon?
  • Have many years of training and experience do you have in the field of plastic surgery?
  • Have you previously performed breast surgeries?
  • What breast reconstruction surgery technique would you suggest in my case?
  • How much time will I be required to commit for the complete reconstruction process?
  • What will be the length of the reconstruction surgery inside the operating room?
  • Which surgical venue would you use for my surgery?
  • Am I a suitable candidate for breast reconstruction?
  • Are the results I am expecting reasonable?
  • Can you show me a few breast reconstruction ‘before and after’ pictures?
  • Where will the surgical scars be located?
  • What type of anesthesia would you use for this surgery?
  • What kind of lifestyle changes and preparation will I have to make before the surgery?
  • What will be the post surgical downtime and how many days of leave should I apply for at my workplace?
  • What are the potential risks and complications that I should know about before undergoing breast reconstruction?
  • Will I have any options for revision surgery if the final cosmetic outcome of my procedure is not as per what was planned?
  • What are the costs of a breast reconstruction, including all the pre- and post-operative costs?
  • Will your office be able to offer me assistance or guidance with regard to my medical insurance claim?
  • Can I review a few testimonials of past patients to learn about their experience with your practice?

Question about the Technique

The patient should ask the surgeon about the choice of breast reconstruction technique. Both breast implants technique and flap reconstruction have certain benefits and limitations. If the patient is unclear about the techniques, they should ask questions and gain clarity from the surgeon.

Question about Tissue Expander

If the patient does not have adequate skin in the breast area to support the implant, the surgeon may suggest a tissue expander technique to support the implant. The patient should ask whether tissue expander will be required or not in her case, and determine whether she is comfortable with the surgeon’s recommended plan.

Breast reconstruction cosmetic surgery is a major decision, requiring serious consideration beforehand. Some breast reconstruction surgeries may involve multiple procedures performed over an extended period of time. shutterstock_107648318 The patient should be mentally prepared to undergo the complete process and have the commitment to follow the surgeon’s instructions throughout. Patients should discuss any concerns and doubts with the surgeon and assess their suitability for the procedure. The surgeon will work closely with the patient to evaluate her candidacy, and help her make an informed choice. Board certified plastic surgeon Dr. Hisham Seify provides breast reconstruction surgery to patients in Newport Beach, Orange County, CA, and surrounding communities.

Characteristics of a Suitable Candidate

Women who have had one or both their breasts removed after breast cancer, are usually ideal candidates for post-mastectomy breast reconstruction surgery. The reconstructive procedure can be performed simultaneously with the mastectomy. Alternatively, a candidate can also choose to have it done after her recovery from breast cancer treatment and mastectomy. Irrespective of when the breast reconstructive plastic surgery is performed and what technique is used, surgeons recommend candidates to have reasonable expectations from the procedure. Patients should know that breast reconstruction will not be able to fully incorporate the look or feel of natural breasts. However, the procedure can be very helpful in restoring a woman’s self-esteem with regard to her general breast appearance. A surgeon will also inform the patient at the outset that not all women who have undergone breast removal will be ideal candidates for reconstructive surgery.

Ideal Candidates

Patients with the following attributes may be ideal candidates for breast reconstruction:
  • A patient who wants corrective surgery to restore the shape and size of one or both breasts
  • A patient who is completely free of cancerous tissue
  • A patient who has responded to breast cancer treatment and mastectomy surgery positively
  • A patient who does not smoke, and does not have diabetes or heart disease
  • A patient who is agreeable to extended time commitments and multiple procedures
  • A patient with realistic expectations

Flap vs. Implant Reconstruction Candidates

For the candidates to choose from surgical processes such as TRAM, latissimus dorsi, DIEP flap, or gluteal, they must not have any such underlying health conditions that may impact the outcome of their surgery. These invasive techniques are more complex than graft reconstruction. It is necessary for the patient to be healthy to sustain the rigors of the procedure. Candidates looking for the restoration of one breast, also known as unilateral breast reconstruction, may be recommended flat reconstruction surgery. This procedure simplifies tissue migration and works well to match the size, look, and feel of another breast. The relatively less invasive nature of implant reconstructive surgery makes it preferable for candidates who are comparatively older. This procedure also requires less recovery time. Patients who need reconstruction of both their breasts or women who lack surplus tissue may choose implant reconstruction. It is essential for patients to realize that breast reconstruction is a major invasive procedure, and should only be performed by a highly experienced and trained surgeon. At the time of initial consultation, the surgeon will educate the patient about all aspects of breast reconstruction, address her concerns and doubts, and help her make a judicious and appropriate decision. Board certified plastic surgeon Dr. Hisham Seify can provide breast reconstruction to patients in Newport Beach, Orange County, CA, and surrounding locations in this part of The Golden State.

Candidacy

Women who have undergone mastectomy for one or both breasts due to breast cancer can be candidates for a breast reconstruction surgery. The right candidate will have the following characteristics:
  • Is interested in restoring her breast shape and size
  • Does not have cancerous tissue
  • Does not smoke or can quit smoking for a certain period
  • Has been able to cope well with cancer treatment
  • Is amenable to multiple surgeries
  • Has realistic expectations from the plastic surgery
The surgeon will explain to the patient the benefits as well as potential risks of breast reconstruction surgery.

Procedure

The patient can choose to go through with breast reconstruction surgery right after the mastectomy or wait until she has healed from the previous surgery. The reconstruction can be carried out using two methods: breast implants or flap technique. The implant method involves placing a silicone or saline breast implant beneath the chest muscle. The flap method of reconstruction involves the use of muscle, fat, or skin from another location on the patient’s body for the plastic surgery. The surgeon will closely guide the patient to ensure that she can choose the technique that is best for her.

Implant Reconstruction

The implant technique of breast reconstruction takes places in multiple stages. At the onset, a balloon expander may be placed beneath the skin and chest muscle to allow the region to stretch. This process can take a few weeks or months till the skin stretches enough to allow the implant to be placed. Then the surgery is performed to position the silicone or saline implant.

Flap Reconstruction

The surgeon will remove extra tissue from an area of the body like the buttock, back, or abdomen and moves it to the chest. The new tissue is used to create the breast mound. If required, and an implant may also be used. There will be more scarring in this method due to additional incisions, which can lead to lengthier healing times. But the results with the flap method may be more realistic looking than the implant technique.

Recovery

The downtime involved in breast reconstruction surgery is typically two weeks. The patient is advised to take short walks and undertake lighter activities within their home, and not to exercise vigorously or lift heavy weights. Profound and prudent cosmetic surgeon Dr. Hisham Seify receives patients from Newport Beach, Orange County, CA, and other neighborhoods and cities in this area of the west coast for breast reconstruction. A woman who has had a single or double mastectomy or lost breast tissue due to an accident may choose breast reconstruction which is a cosmetic surgery procedure to restore the shape and size of breasts. Breast reconstruction can be performed after a mastectomy or at a later date.iStock_000009836956Small During the pre-op consultation, the surgeon will provide the patient with information on various facets of breast reconstruction and the costs associated with this procedure. Factors Impacting the Financial Implications

Many factors can influence the sticker price of breast reconstruction surgery. Price tags can vary from one practice to another, and even between two patients at the same practice in some cases. Location of the Practice In metropolitan cities and prime urban areas, the cost of breast reconstruction plastic surgery will typically be slightly higher. Conversely, smaller cities and areas in the Midwest may have lower prices due to lower living costs. This is not really shocking. This is the way it is for most items. For example, a pickup truck is much cheaper to buy in Oklahoma than California for a variety of reasons, lower taxes is also one of them. Surgeon’s Charges An experienced surgeon with high qualifications may charge more than a new or less experienced surgeon working under the guidance of a senior surgeon. It is advisable to choose board certified surgeons due to their focus on high standards of care, advanced surgical methods, and ongoing education which produces excellent results for patients. Surgical Venue Costs The breast reconstruction surgery can be performed in a hospital, an accredited surgical center, or a private suite. The location of the surgery is also dependent on whether the surgeon has operating privileges at a local hospital or surgery center. Authorized surgical centers are often a preferred location due to their safety standards being akin to hospitals. The patient should evaluate all parameters related to costs and benefits before arriving at a decision.

Surgical Technique

Breast reconstruction plastic surgery typically involves implants. In certain cases, only fat transfer augmentation or flap technique may be involved. The costs also depend on whether the surgery is performed along with the mastectomy or later.

Insurance

Breast reconstruction surgery should be included in the patient’s insurance plan, irrespective of how many years after the mastectomy or lumpectomy the reconstructive surgery is required. This also includes surgeries needed to enhance the reconstructed breast or create a more symmetrical appearance between the two breasts. According to the Women’s Health and Cancer Right’s Act of 1998, all group health insurance plans paying for mastectomy are also required to pay for prostheses and reconstructive surgeries.

Financing

Professional medical care financing firms provide many competitive financing plans. Loans on low-interest rates are available from these firms for patients that do not have insurance coverage. Some other options that a patient may consider include credit cards, bank loans, and even asking to borrow some money from family and/or friends. Patients should consider these options in case they have no insurance which they most likely do not in this case. Once the surgeon approves, the next step should be consult with an experienced breast reconstruction surgeon. When these consultations are held before mastectomy, it will allow the surgeon to come up with the most suitable surgery plan for the patient. Benefits of Breast Reconstruction

Breast reconstruction surgery offers several benefits. Some of the key benefits include:
  • Symmetrical appearance of breasts in tight-fitting clothes
  • Permanent restoration of breast shape
  • Replacement of synthetic prosthesis with biological breast
  • Regaining personal confidence and a sense of self-worth
As with any surgical procedure, breast reconstructive surgery will also leave a few scars behind (that would eventually fade over time.) But with the help of latest surgical techniques, a skilled surgeon can minimize scarring. After a mastectomy, a breast reconstruction will help most patients feel better about their appearance. While wearing a bra or form-fitting clothes after the surgery, the breasts will look and feel alike.

Types of Breast Reconstruction Procedures

Several types of breast reconstructive procedures are available these days. But some techniques require multiple procedures over an extended period. The patient should be able to commit the required time in order to have a safe and successful surgery. A candidate should discuss every aspect of the surgery with the surgeon carefully to make a well-informed decision. Two major surgical techniques that are used for breast reconstruction include:
  • Breast implant surgery performed with the help of silicone or saline breast implants
  • Tissue Flap procedure that uses body’s own tissue
While in most cases, these techniques are performed individually, sometimes the surgeon may combine the two for a more effective breast reconstruction. Additionally, the surgeon may also perform fat grafting to create the appearance of remodeled breasts close to actual breasts. They may also reconstruct the areola or nipple that can complete the look of a breast. However, the nipple areola complex will not have any sensation.

Reconstruction after Lumpectomy or Partial Mastectomy

Women that undergo lumpectomy or partial mastectomy are usually not required to go for breast reconstruction. But in some cases, the patient may have to live with misshaped breasts until they choose a different type of reconstructive surgery to restore the original shape. The surgeons can perform fat grafting or smaller implants as per the patient’s needs. The patient should discuss the risks, benefits, and suitability of the breast reconstructive surgery with her surgeon before committing to the procedure. As with any surgical procedure, breast reconstruction has its own risks. The key to a safe procedure lies in making a well-considered choice of surgeon. Breast Augmentation Plastic Surgery

Breast Augmentation Plastic Surgery Cost, Recovery Newport Beach | Orange CountyBreast augmentation cosmetic surgery procedure can help a woman achieve her aesthetic desire to have bigger and fuller breasts. Apart from aesthetic purposes, breast augmentation may also be performed as a reconstructive surgery following mastectomy or breast injury. Board certified plastic surgeon Dr. Hisham Seify is a leading provider of breast augmentation and various other procedures of the breast, body, and face. Dr. Seify receives patients from Newport Beach, Orange County, CA, and surrounding communities.

Types of Breast Implants

Breast augmentation plastic surgery can be performed using saline or silicone gel based breast implants. Saline implants have an outer silicone shell, while the inner core is filled with a saline solution. In case of silicone implants, the inner core includes a plastic gel called silicone. Both types of implants have been approved by the FDA. Saline implants do not pose any risk in the event of implant leakage because saline solution is completely biocompatible. However, silicone implants are more in demand today because of the more natural touch and feel they can provide. Advanced qualities in silicone implants use a highly cohesive silicone gel that will not leak out to other areas even if the implant shell is damaged or broken.

Surgery and Your Checking Account

As an experienced cosmetic surgeon, Dr. Seify will discuss all aspects of breast implant procedure with the patient, including its estimated costs. He provides breast augmentation to patients in Newport Beach, Orange County, CA, and nearby locations. The average cost of breast augmentation surgery can vary from $7,000 to $15,000, depending on various factors. Patients may choose to finance the procedure through medical care financing companies or other sources. Insurance may cover full or partial costs in case where breast augmentation is a part of a breast reconstruction procedure.

Procedure

In many cases, breast augmentation can be performed as an outpatient surgery. The procedure is usually completed in less than two hours, and patient is likely given general anesthesia. The procedure will begin with the surgeon placing an incision in a pre-determined location below the breasts, around the nipples or under the arms. An implant pocket will be created surgically through the incision. If the procedure involves saline implants, the incision can be relatively smaller because the implant can be inserted in a deflated state. The surgeon will fill the required amount of saline solution after the implant is placed. In case of a silicone implant placement, the size of the incision will be a little larger, but the surgeon can place it in a discreet location to minimize the scar visibility. The implant will be placed into the breast pocket below or above the chest muscle. After the implants have been adjusted into a desirable position, the surgeon will close the incisions carefully with sutures or surgical adhesive.

Recovery

In the first week, the patient will experience mild pain, which can be managed with pain medications. Bruising and swelling will reduce significantly in the second week. Patients can usually return to work in about 10 to 14 days.

Breast Implant Filling Recommendations

Breast augmentation cosmetic surgery has emerged as the second most popular aesthetic surgery procedure after liposuction in the US. Improvements in the quality of breast implants have played an important role in making the procedure safer and more predictable. During the initial consultation for breast implant surgery, the plastic surgeon will explain all aspects of implants, and help the patient make the right choices. Board certified plastic surgeon Dr. Hisham Seify is a well-known name in the field of breast surgery. Dr. Seify will discuss the pros and cons of both saline and silicone implants, and evaluate the appropriate size or volume options with the patient. He receives patients from Newport Beach, Orange County, CA, and surrounding communities.

Filling of Saline Implants

Breast augmentation plastic surgery may be performed using saline or silicone gel based implants. While silicone implants come prefilled, and the volume cannot be adjusted later, but saline implants are deflated at the time of their surgical insertion into the breast tissue. Thereafter, the surgeon will fill the precise volume of saline liquid to create the appropriate breast size as needed by the patient. It is important to choose a qualified and experienced cosmetic surgeon for breast implant procedure to ensure that the most effective choices are made in terms of breast implant type, size, profile, and texture, apart from incision placement and the position of implants. Patients in Newport Beach, Orange County, CA, and nearby locations have an opportunity to receive breast implant surgery from Dr. Seify.

Recommended Volume

Saline breast implants are identified by their lowest recommended volume. However, a majority of surgeons will fill the implant to its highest recommended volume. This should not be confused with the issue of “overfilling” of the implant. When a breast implant is filled to its highest recommended volume, it will usually perform better. The feel of the implant will improve, and the chance of implant failure will reduce. An “underfilled” implant means that the implant has not filled up to its lowest recommended volume range, and an “overfilled” implant is the reverse of this condition. In both overfilled and underfilled implants, the risk of implant deflation, rippling, or a ball-like effect will increase. If the volume is more or less by at least 15% than the recommended volume, the risk of implant failure goes up significantly.

Volume and Implant Profile

Implant profile is a vital aspect of the overall aesthetics of breast implants. At the time of pre-op planning and preparation, the surgeon will determine the most desirable size and profile of implants in accordance with the patient’s breast anatomy and her personal aesthetic needs. The implant profile will be impacted by the amount of volume in a saline implant. Recommended volume ranges in saline implants are defined on the basis of the width of the base of the implant. In case of wider breast implants, a larger volume can be filled. This is typically true for implants with a moderate or medium profile. The fill range becomes much higher in case of high profile implant devices as compared to moderate profile implants. Breast Augmentation Asymmetry

Cosmetic Surgery | Breast Implant Asymmetry | Newport Beach | Orange Country CABreast augmentation cosmetic surgery may sometimes lead to a condition called breast implant asymmetry. In this condition, the breast implants fail to settle in the desired position on the chest wall, or settle in different positions. In most cases, the implant symmetry is minor and does not require a corrective procedure. The asymmetry will usually not be detected in bras or clothes, but may be noticeable when the woman closely examines her breasts in the mirror. However, in a few cases, the asymmetry may be more conspicuous, requiring a secondary procedure by a plastic surgeon. Dr. Hisham Seify is a board certified plastic surgeon providing corrective surgery for breast implant asymmetry. Dr. Seify receives patients from Newport Beach, Orange County, CA, and surrounding communities.

Types of Implant Asymmetry

Upper pole fullness If the breast implant fails to descend appropriately, an upper pole fullness may occur, which will leave the implant too high. Upper half fullness of the breast will occur, which can appear unnatural. A relatively simple upper pole fullness corrective plastic surgery will be performed in such a case. The implant on the high side will be repositioned downwards by the releasing the scar tissue capsule. Scar tissue release on the affected side will be done by cutting on the lower half of the scar tissue capsule. The implant’s weight will then rest on the new lowered position. The degree of scar tissue release will depend on the size of the implant or how much of breast implant lowering is required. Lower pole fallout If the breast implant settles very low, then lower pole fallout can occur. This can be more difficult to correct. This condition can occur due to the weight of the implant, the quality of the breast tissue, or if the cosmetic surgeon made the original implant pocket too low. If the pocket is very low, the inframammary breast fold may be released too much. The result will be reverse of the upper pole fullness in such a situation. The condition can be corrected with a breast revision procedure. Reconstruction of the breast fold will be performed on the involved side. Following the surgery, the patient will be required to wear a support bra for about four weeks to enable the fold to reform and heal. Dr. Seify provides breast revision and other procedures to patients in Newport Beach, Orange County, CA, and other areas. Misdirected Nipple This type of implant asymmetry involves malpositioning of the nipple over the breast mound. The nipple areolar complex should ideally be close to the most projecting point of the implant, and on the breast mound. The choice of breast implant size should be made while taking this aspect into account. If the implant is poorly positioned, the nipple may be misdirected. When the implant has adopted different positions on different sides, the nipple will point in different directions. Patients should note that it is normal in many cases to have nipples at different levels. If the patient is made aware of this fact at the pre-op stage, she will have realistic expectations and will feel satisfied with the final results because she will have a better understanding on what to expect.
